2. Add in a Title: (YourNickName UNBAN)
3. In the message area, answer the questions provided bellow.

Form

• Your nickname in-game.
• Server you were banned from.
• Upload files (attach them) or put the link. (demo+screenshots in one .rar file) you can use https://zippyshare.com or any other files uploader.
• Comments. (Describe the situation or put your complaints)
• Information from console. (copy/paste BAN information which was printed in your console)
